Event Selection & Insurance Instructions

 

 

 

Event Selection:

 

The sponsor level page lists all events & attractions at Unity Day.  Some events have already been spoken for.  If the event has a committed Sponsor listed next to it, the event is not available.

 

Please call Caesar Mistretta Jr. (908)-766-6400 to find out the availability and pricing of events.

 

We will have 30 + vendor (business/organization) tents available, so far 10 tents have been spoken for. 

 

 

Required Forms:

 

(1) Organization/Business Activity Application:   This application is filled out by the vendor.  Please fill in all of the information.

 

(2) Certificate of Liability Insurance:  This form (see SAMPLE) must be filled out and sent to you by your insurance company.  The following information must be on the form or the Borough will not accept it.

 

1)      Producer: Insurance Co.

2)      Insured: Name of insured

3)      Policy number

4)      Policy effective date & expiration

5)      Description:  The Borough of Bernardsville and The Bernardsville Chamber of Commerce are named as an additional insured. Unity Day, September 20, 2008 with a rain date of September 21, 2008

6)      Certificate Holder:  The Borough of Bernardsville, 166 Mine Brook Road, Bernardsville, NJ & The Bernardsville Chamber of Commerce, PO Box 672, Bernardsville, NJ 07924

 

(3) Hold Harmless Agreement:  This form (ATTACHED) is filled out by those vendors having a tent.  Please fill in all of the information and have it signed and if you are a corporation have it notarized by a notary.

 

PLEASE NOTE:

 

PLEASE RETURN ALL COMPLETED PAPER WORK:

 

(1) Application,

(2) Certificate of Liability Insurance (if you have a tent),

(3) Hold Harmless Agreement (if you have a tent), and

(4) Check for appropriate amount
